The transition over time ❊

It starts small
A few extra appointments. More check-ins. A little more help than before.

Then it gets more complicated
Decisions, paperwork, healthcare, housing, changing needs. Suddenly you're managing things you never expected to manage.

02

That’s where we come in
Shannon helps families navigate these transitions with experience, practical support, and a steady hand when it's needed most.

03

Because sometimes what families need most isn't another service. It's someone who understands.

Practical help,

We used to offer a few different plans, but experience has taught us that a more holistic approach is simply a better way to serve our clients.

THE QUARTERLY HOME CONCIERGE PARTNERSHIP
$9,550.00

The Home Concierge Partnership

When life changes for your parents, it changes for you, too.

As our parents get older, there can be more to manage than we ever anticipated. Sometimes it's a gradual change in their ability to manage everyday life. Other times, a diagnosis, a hospital stay or an unexpected event means making important decisions sooner than anyone expected.

And while you want to be there for them, you're also managing a life of your own.

That's where Shannon comes in.

The Home Concierge Partnership brings experienced, personal support to families navigating the changing needs of an aging parent. From understanding care options and coordinating appointments to managing the home and planning for what's next, Shannon helps make sure the important things are taken care of, with the attention and follow-through you'd give them yourself.

It starts with a conversation.

Every family has its own circumstances, priorities and questions. Before recommending anything, Shannon takes the time to understand your parent's needs, their wishes and what your family is hoping to achieve.

From there, she'll develop a written plan outlining recommended next steps, available options and anticipated costs. It's a practical starting point that gives everyone a clearer understanding of what needs to happen and how to move forward.

And because circumstances change, the support changes with them.

How Shannon can help.

Navigating health and changing care needs

Whether your parent is experiencing memory changes, living with dementia or simply needs more support than they once did, Shannon can help you understand the options and coordinate the people and services involved.

This includes:

  • navigating dementia and cognitive changes

  • coordinating appointments

  • communicating with healthcare providers

  • supporting hospital discharges

  • helping families respond when unexpected situations arise

She can also advocate for your parent's needs with care teams and Ontario Health atHome, making sure important conversations happen and nothing gets overlooked.

Making decisions about home and living arrangements

For many families, the goal is to help a parent remain comfortably and safely in their own home for as long as possible.

Shannon can assess what additional support may be needed:

  • coordinate home safety improvements

  • oversee trusted trades and service providers

When a change in living arrangements becomes necessary, she can help families:

  • explore retirement residences and long-term care options

  • arrange visits, navigate applications

  • coordinate the practical details of downsizing and moving

Bringing the right professionals together

Planning for an aging parent's future often involves legal, financial and healthcare decisions that need to be considered together.

Shannon works alongside your family's existing professional advisors, or helps connect you with trusted specialists, to coordinate matters such as powers of attorney, estate planning, financial arrangements and the anticipated costs of care.

Supporting the family, too

Caring for an aging parent can bring questions and decisions that aren't always easy to navigate, particularly when siblings live in different cities or have different perspectives.

Shannon offers practical guidance, helps families work through their options and can arrange additional caregiving or respite services when needed.

With regular communication and updates, everyone can stay informed without having to personally manage every detail.

Looking after everyday life

Sometimes it's the smaller things that make the biggest difference.

Home maintenance, seasonal organization, appointments, social activities, special occasions and the countless details that help a home run smoothly.

Shannon can coordinate it all, helping your parent continue to enjoy the comforts and routines that matter to them.

A partnership that grows with your family.

The real value of the Partnership is continuity. Having someone who knows your parent, understands your family and is familiar with the decisions you've already made.

Someone who can anticipate what might be needed, follow through on the details and be there when circumstances change.

For your parent, it means thoughtful, personal attention. For you, it means knowing there's someone you trust looking after the things that matter.

Your next chapter,
told by others

She understood before I even knew how to explain it.

"Mom wasn't ready for long-term care, but she wasn't really managing on her own anymore either. I spent months feeling guilty and overwhelmed because I couldn't figure out the next step. Shannon somehow understood exactly where we were without me having to explain every little detail. She never rushed us, never pushed us, and helped us make decisions one at a time. She gave our whole family permission to breathe again."

She knew exactly who to call at just the right time.

"We needed everything at once. Trades, movers, paperwork, cleaners, donations, and repairs before we could sell my parents' home. I don't know how we would have managed without Shannon. She had trusted people for everything, coordinated it all, and saved us countless hours and probably thousands of dollars. More importantly, she let us spend our time with Dad instead of chasing contractors."

— Patrick M., Son in law of client

She understood dementia in a way most people don't.

"When dementia became part of our story, every day felt different. Shannon never treated Mom like a problem to solve. She met her exactly where she was. She understood the confusion, the emotions, and even how to communicate in ways that helped Mom feel safe. I never had to explain why certain things mattered. She simply understood."

— Joe W., Son

She made an impossible system feel manageable.

"Trying to understand long-term care was overwhelming. The waiting lists, applications, assessments, timelines... we had no idea where to begin. Shannon walked us through every step, answered questions we didn't even know to ask, and helped us make informed decisions without feeling pressured. Having someone who truly understood the system gave us confidence during one of the hardest seasons of our lives."
